Admissions Process

Referrals are welcome from any source including hospice, home care agencies, hospitals, physicians, social workers, family, friends or the person themselves. The first step is always just giving us a call (712) 244-3020 or by stopping in for a tour with the Executive Director or House Manager. 

Become a Resident of Mitchell House

Referral and Eligibility

  • A referral is made for a person who needs extra support in a home-like setting. This is often initiated by the hospital care team, a hospice provider or the individual’s physician. 
  • Referred person(s) must be affiliated with a hospice agency. We suggest that you check with your physician in order to start that process. Each resident must be working with a hospice agency of their choice

Selection of Services

  • While Mitchell House provides the living environment, the resident’s care is coordinated with the hospice provider and staffing agency of choice. 

Coordination Between Hospital, Care Providers, and Mitchell House

  • Mitchell House staff works directly with the referral source to arrange a smooth transition to Mitchell House. 
  • Mitchell House will prepare the resident’s room and make sure all needed services with the hospice provider and the staffing agency are in place before arrival.

Move-In Process

  • Transportation of the resident to Mitchell House is arranged and may occur by family car, wheelchair service or non-emergency ambulance if needed. 
  • Upon arrival, Mitchell House staff welcome the resident and family, help them get settled, and review important information from the Resident Welcome Packet.
  • Family is welcome to spend the night in the adjacent family suite with queen-sized sofa, kitchenette with minifridge, closet, table and chairs.

Mitchell House's Role in Care:

  • Mitchell House provides residential services such as a hospital bed and linens, lift chair, meals, coffee bar, laundry and housekeeping in a a peaceful, home-like setting. 
  • Volunteers are available to provide companionship to residents such as playing cards or reading books. You will also find them preparing snacks such as home baked cookies or tending to the outdoor flowers. 

Hospice Provider's Role in Care:

 Hospice develops the resident’s plan of care based on the assessment completed by the hospice nurse and team members, in accordance with physician orders and the resident’s and family’s wishes. 

 Hospice will provide the following as indicated in the plan of care:

  • Medications for pain and symptom management
  • Syringes for liquid pain medications
  • Personal protective equipment 
  • Wound care supplies
  • Oxygen and related supplies
  • Foley catheters and incontinence supplies
  • Medical equipment such as a wheelchair, shower chair, bedside commode, etc. 
  • Team visits according to care plan 
  • Hospice will maintain records of the following: physician orders and certification statements, care plans, documentation of care provided, family contact information, funeral arrangements, care at time of death and any other pertinent resident information

The hospice/nursing agency will maintain all record and ownership of the following: physician statements, care plans, funeral arrangements, and emergency/familial points of contact.

Staffing Agency's Role in Care

 

  • Provide assistance with personal care, meals, and medications in accordance with the hospice plan of care.
  • Maintain a clean and safe environment. 

Important Notes:

  • Families are encouraged to visit anytime, day or night. 
  • Mitchell House does not supply personal items like toiletries or clothing, so families should bring anything their loved one will need.
  • Meals are homecooked and tailored to residents’ needs. Families are welcome to bring favorite foods to share and use the community kitchen to cook meals. Grills are also available for use on the outdoor patios. 

Admission Instructions for Medical Professionals

To start the referral process, fax the following information to 712-244-3024 and call the facility at 712-244-3020 

  1. Face sheet with demographics
  2. Medical history with current notes
  3. DNR
  4. Current medication list
  5. DPOA name and contact information
